8/11/2023 EnchantaVaughn Approved Behavior: During in home hours, Anthony exhibits great participation, following rules and expectations without hyperactive conduct. Anthony was supported for increasing his involvement in social and recreational opportunities. Anthony continues to execute responsibility for his actions by making good choices and utilizing all strategies in session today. Anthony was cooperative and polite, presenting safe behaviors. Behavior: Anthony displayed off-task behaviors evident by him starring at the walls and wanting to play with his sibling during session. Once he was redirected by QMHP-C he was able to regain focus of the session. Behavior: Anthony required redirection from QMHP-C as Anthony was screaming to the top of his lungs when he was unable to have his needs met. Intervention: QMHP-C motivated Anthony to respond to exercise and express his feelings in front of his mom. QMHP-C provided Anthony with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP-C encouraged Anthony to communicate to his mom when he feels down to prevent unwanted depression. Intervention: QMHP-C praised Anthony for staying on task. QMHP-C explored the positive behavior options he was exhibiting with Anthony to support his willingness to continue to make good decisions and have positive behaviors in at home, in school, and in the community. Intervention: QMHP-C complimented Anthony for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP-C supported Anthony displaying positive social interaction with peers and authority. QMHP-C praised Anthony for listening and following directions and interacting safely during each intervention. QMHP-C observed Anthony complying with rules and expectations. QMHP-C offered active listening skills. QMHP-C assisted Anthony with recognizing his own personal strengths and how to apply them to reach his goals and objectives daily. Response: The frequency and intensity of the Anthony's defiant affect decreased. Anthony engaged in reflective thinking and expressed his feelings to QMHP-C. Anthony remembered to apply himself and utilize his strengths to focus on meeting the goals listed on his ISP. Anthony was acceptive of encouragement and decided to listen to his mother and do the right thing. Response: Anthony was seen following desired rules and expectations without requiring redirection or additional breaks. Anthony realized his mother was trying to help him by pushing him to try on his own first and he would correct his mistakes as needed. Anthony promised to do his work and make something out of himself. Response: Anthony continued to work on the psycho-educational activity and implement a more alert persona while contributing to one on one discussion. Anthony was able to process and reflect on his day with QMHP-C. Anthony listed 3-4 behaviors that he was going to utilize in efforts to remain positive in the home. Anthony appeared to be receptive to receiving positive reinforcement from his mother, as demonstrated by him smiling and warm inviting body language. Anthony was able to recognize his own personal strength so that he could achieve his goals listed on his ISP.
